Secure a Copy of a Certificate of Insurance Previously Issued

Please email a request to riskmanagement@archbalt.org including the name and address of the Certificate Holder for which you are seeking a copy.  Be sure to advise the location name and 4 digit code and any other helpful information to assist us in providing you the requested document.
