• Request for Permission for Filming and Release Agreement – If a person or company wishes to film at any parish, school or other Archdiocesan location, the location must complete this form and submit it to the Archdiocese Communications Office for review and approval.

Standard Short Form Agreements

• Outside Services – Short Form Agreement to be used when parishes or schools want to contract for services such as mowing, landscaping, cleaning, snow removal or other services that are provided at either regular intervals or conditioned upon needs such as snow removal. (updated 2023.02)  The services provided by the vendor can either (a) be described in Paragraphs 1 through 3 of the agreement or (b) be described in a proposal attached to the agreement, with the proposal being referred to in paragraphs 1 – 3.
